What is POLY-B Injection?
Poly-B contains the active ingredient Polymyxin B sulfate. Polymyxin B sulfate is an antibiotice used in the treatment of several different infections caused by susceptible strains of Ps. aeruginosa including urinary tract infections (UTIs), meninges, bloodstream infections and eye infections. Poly-B is an injectible form of this antibiotic.

How is POLY-B Injection?
Poly-B is a liquid. It comes in a vial containing 500,000 units of Polymyxin B Sulphate. This solution will need to be diluted prior to use. The amount of dilution will depend on the method of administration.

If you are using Polymyxin B Sulphate intravenously, one vial will need to be dissolved in 300mL to 500 mL of a suitable solution for continuous drip. A dosage of 15,000 to 25,000 units/kg body weight/day is usually given to adults with no kidney problems. This dosage will be adjusted for adults with kidney impairment or to children or infants.

If you are using Polymyxin B Sulphate via intramuscular injection, the vial hould be dissolved into 2 mL sterile water for injection or into sodium chloride injection or procaine hydrochloride injection 1%. A dosage of 25,000 to 30,000 units/kg/day should be given to healthy adults. Children and patients with renal problems require an adjusted dosage. Note: this method of administration should only be performed in a hospital environment by medical staff.

If you are using Polymyxin B Sulphate via Intrathecal Injection, the contents of one vial of Poly-B should be dissolved in 10 mL sodium chloride injection USP. A dosage of 50,000 units once daily is given for 3 to 4 days. This is then reduced to 50,000 units once every second day for at least 2 weeks after cultures of the cerebrospinal fluid are negative and sugar content has returned to normal. Note: this method of administration should only be performed in a hospital environment by medical staff.

If you are using Polymyxin B Sulphate as an opthalmic treatment, the vial needs to be dissolved in 20 to 50 mL sterile water for injection or sodium chloride injection USP. Apply drops of this new solution 1 to 3 times per hour.

Adverse effects
Notify your doctor if you notice any of these side effects that have been reported with use of Polymyxin B Sulphate:

  • albuminuria
  • cylinduria
  • azotemia
  • facial flushing
  • dizziness progressing to ataxia
  • drowsiness
  • peripheral paresthesias
  • fever
  • headache
  • stiff neck
  • urticarial rash
  • severe pain at intramuscular injection sites
  • thrombophlebitis at intravenous injection sites

There is a nother potentially fatal side effect of Polymyxin B Sulphate called Clostridium difficile associated diarrhea (CDAD) or Antibiotic-Associated Diarrhea (AAD). Any diarrhea you experience while taking Polymyxin B Sulphate should be reported to your doctor immediately.

Warnings
Impaired renal function can cause complication with Poly-B Injection. Any patients with renal problems must be monitored at all times while taking Polymyxin B, and the dosages will need to be adjuested.

Intramuscular injection or intrathecal injection must only be performed on hospitalized patients by medical staff.

Polymyxin B Sulphate has known drug interactions with bacitracic, neomycin, streptimycin, kanamycin, tobramycin, gentamycin, amikacin, cephaloridine, paromomycin, colistin, viomycin. If you are taking these or any other neurotoxic or nephrotoxic drug.
